An Atrius Health Equity Foundation Grant
Atrius Health's "City's Live Long and Well Agenda Awards", will enable the Dorchester Food Coop to provide monthly free food boxes to 500 Uphams Community Care patients who are at high risk for cardiometabolic health issues. The boxes will include produce, grains, and protein, and be customized to the needs of patients with high blood pressure, diabetes, and other diagnoses with specific dietary conditions. All of these patients will be strongly encouraged to sign up for mobility coaching, allowing us to track the results of a combination of wellness/wallet interventions.
The grant would also cover the cost of 500 Solidarity Shares ($100 one-time memberships) to the Dorchester Food Coop. Members will own their share for their lifetime. They receive monthly discounts and other benefits, and are encouraged to run for the Board of Directors and participate in the governance of the Co-op. The Solidarity Shares will be provided as benefits to coaching clients – incentivizing participation while contributing to better cardiometabolic health.
